I understand that the Ottawa Athletic Club has a NO REFUND policy and agree to pay the noted sum to cover the cost of the membership whether or not I avail myself of the privileges of the membership granted to me. I affirm and understand that there are inherent risks involved in all club activities, and that I am aware of and appreciate these risks. I assume full responsibility for myself and those on my membership for any and all injury, loss or damage, including costs of any lawsuit, loss or theft of personal property or personal injury. I release and indemnify the Ottawa Athletic Club, OAC Holdings Ltd., and
their agents, owners, officers, directors, volunteers, employees, contractors, and all other associated persons or entities acting in any capacity, and will not hold them responsible in any way if I or any people on my membership are injured or incur loss for any reason whatsoever. I confirm that I am physically able to participate in the activities of the Ottawa Athletic Club. I give my permission for the club to contact me regarding program or membership information and to photograph me for possible promotional purposes.
